[City of Baraboo] Operator License application NEW and Provisional

All Applicants Please Note: The City of Baraboo has recently amended their Ordinance regarding Intoxicating Liquor and Fermented Malt Beverage. With this change, ALL applications submitted for an Operator's License, both Regular and Temporary, must be reviewed and approved by both the Administrative Committee and the City Council. Because of this, please allow 45 days for processing.

Hello applicant. You are about to apply for a BARTENDER’S LICENSE also known as OPERATOR license. Here are a few pointers to help you along the way. If you already have a license previously issued by the City of Baraboo, or need a temporary operator license to work at a special event, this is not the application for you. Please complete the OPERATOR LICENSE APPLICATION FOR RENEWAL OR TEMPORARY OPERATORS.

If you are a new applicant, gather one of the following documents which you will be asked to submit with your application. 1. Certificate of completion of the Beverage Server Training Course dated with the last 2 years. 2. Copy of bartender’s license issued by another Wisconsin community dated within the last 2 years.

You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference. You must be 18 years of age to apply. A record check will be conducted for all applicants.

Payment of $87 or $97 is required at the end of the process depending upon how you apply. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
