Hello applicant. You are about to apply for a license to operate a taxi service. Here are a few pointers to help you along the way. For questions, please refer to Baraboo City Code 12.11 available on the City’s website at WEBSITE.

Here are the documents that you will be required to submit along with your application. Please have them ready to upload into your application: · Insurance Certificate in the amount of $500,000 for any one accident, $500,000 personal injury, and $100,000 property damage. · Vehicle Inspection Report from a qualified service technician.

Before starting the license application, you will need to have your vehicle(s) inspected by a qualified technician. Here is a form you can use to document the inspection. After the inspection is complete, scan and upload it at the end of your application. Taxi Inspection Form

You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference.
Payment of $10 per vehicle is required at the end of the process. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
