[City of Baraboo] Vicious Animal License Application

Hello applicant. You are about to apply for a Vicious Animal license. Here are a few pointers to help you along the way. For questions, please refer to Baraboo City Code 12.13 available on the City’s website at WEBSITE.

Here are the documents that you will be required to submit along with your application. Please have them ready to upload into your application: 1. Insurance Certificate in the amount of $300,000 personal injury, $50,000 property damage. 2. Confirmation of permanently marking the animal a subcutaneous microchip and providing the identification information

We will confirm the animal is spayed or neutered and the rabies certification by reviewing the current license with the City.

You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference.

Payment of $100 is required at the end of the process. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
