Hello applicant. You are about to apply for a Fireworks User license. Here are a few pointers to help you along the way. Before beginning this application you should gather the following documents which you will be asked to submit with your application. 1. Contract from the fireworks supplier. 2. Certificate of Insurance in the amount of $1,000,000 holding the City of Baraboo harmless.

You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference.
Payment of $150 or $15, depending upon the applicant, is required at the end of the process. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
