[City of Baraboo] Animal Rescue application

Hello applicant. You are about to apply for an Animal Rescue Operation license. Here are a few pointers to help you along the way. For questions, please refer to Baraboo City Code 12.13 (12A) available on the City’s website at City of Baraboo.

Before beginning this application you should gather the following documents which you will be asked to submit with your application. 1. Proof of membership with Alliance of Wisconsin Animal Rehoming Efforts. 2. Letter of recommendation for Doctor of Veterinary Medicine supporting animal rescue operation. 3. Evidence that rescue organization is a bona fide animal rescue organization.

You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference.

Payment of $125 is required at the end of the process for a new application, or $100 for a renewal application. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
