[City of Baraboo] Operator License application Renewal and Temporary

All Applicants Please Note: The City of Baraboo has recently amended their Ordinance regarding Intoxicating Liquor and Fermented Malt Beverage. With this change, ALL applications submitted for an Operator's License must be reviewed and approved by both the Administrative Committee and the City Council. Because of this, please allow 45 days for processing.

Hello applicant. You are about to apply for a BARTENDER’S LICENSE also known as OPERATOR license. This application is used for Renewal of your current operator's license or to apply for a temporary license. Here are a few pointers to help you along the way. You will not be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference. You must be 18 years of age to apply. A record check will be conducted for all applicants.

If you do not have a current operator license issued by the City of Baraboo, do not fill out this form. Instead use the "OPERATOR-NEW" application.

Payment of $87 (renewal) or $5 (temporary) is required at the end of the process. Payment options include: online, US mail, or in person. More details will be provided at the completion of the application.
