Hello applicant. You are about to apply for an ANNUAL MOBILE FOOD VENDOR PERMIT.

Here are a few pointers to help you along the way.

Gather the following documents to help you complete your application.
1. Your vehicle's valid Proof of Registration
2. Names, dates of birth, addresses and copies of valid Driver's Licenses for all Operators of the vehicle.
3. Valid License issued by Sauk County Health Department
You are required to attach a copy of each of these to your application

You will be able to save an incomplete application. If you have to quit before finishing you have the ability to print the incomplete application for later reference.

You must be 18 years of age to apply. A record check will be conducted for all applicants.

Payment is required at the end of the process. Payment options include: online, US mail, or in person.
More details will be provided at the completion of the application.
After completing application and payment, please allow 15 days for processing.
